In order to help make your upcoming allergy appointment with us as helpful as possible, please take a few moments to fill out the enclosed questionnaire.
When we see you, we will review your questionnaire with you and examine you. If skin testing is to be performed on the day of your appointment, please do not take any medications containing antihistamines for a period of at least four (4) days before the appointment. (Antihistamines are medications taken for nasal problems or itching and include Allegra, Claritin, Clarinex, Zyrtec, Benadryl, Contact, Dristan, and many other over-the-counter, as well as prescription medications). A few medications such as Sinequan, Elavil, and Tofranil need to be discontinued for even longer periods before testing. (Please call us if you have taken any of these in the past two weeks.)
Other medications for asthma, high blood pressure, heart problems, thyroid, diabetes, and other medical conditions should be taken as you regularly do, without any need to discontinue them. Please feel free to call us if you have any questions or if you are unsure about any medications that you may need to stop before your appointment.
It can be helpful to obtain a copy of any previous allergy evaluations. You can contact your previous physician and either have them mail the records to us or bring them with you at the time of your appointment. Alternatively, you can sign a release of records at our office and we will obtain the records for you.
As a reminder, a parent or guardian must accompany minors seen for their initial consultations.
Please remember to arrive at least 20 minutes early for your appointment in order to fill out additional paperwork and avoid unnecessary waiting. We are located at 24120 Meadowbrook Road, Suite 201, Novi, Michigan in the Tobin Professional Plaza (on the east side of Meadowbrook Road, just south of Ten Mile Road).

NO SHOW POLICY
We strive to create as many appointments as possible for the physician and nurses to provide all the services needed for our patients. We need your help to make this system work. We know and understand how busy your lives are and we know that plans change. We would like the simple courtesy of a telephone call if an appointment cannot be kept.
It is our policy that any prescheduled appointment be cancelled at least 24 hours in advance except in case of an unforeseen emergency.
If an appointment is cancelled, we will do our best to give you the next available appointment time for the type of visit required. Since we reserve a longer period of time for new patients, if you are a new patient and do not keep or cancel your appointment with your physician, you will be charged $50.00. Our existing patients will be charged $25.00 for missed physician appointments without prior cancellation.
